The Health Insurance Portability and Accountability Act of 1996 (HIPAA), Public Law 104-191, was enacted on August 21, 1996. Sections 261 through 264 of HIPAA require the Secretary of HHS to publicize standards for the electronic exchange, privacy, and security of health information. The Privacy Rule standards address the use and disclosure of individuals' health information—called "protected health information" by organizations subject to the Privacy Rule — called "covered entities," as well as standards for individuals' privacy rights to understand and control how their health information is used. Within HHS, the Office for Civil Rights ("OCR") has responsibility for implementing and enforcing the Privacy Rule concerning voluntary compliance activities and civil money penalties.
Long Life Care of Vermont is a company that abides by HIPAA regulations. The company is strictly prohibited from sharing personally identifiable information of clients to a third party. No employee of Long Life Care of Vermont is permitted to share personally identifiable information of clients on and off duty unless it is authorized by the clients to help us understand how we can better serve the clients.
We may collect personal information when you register for an account, use any of our online services, book an appointment, make a donation, on our websites, or submit documentation using forms (such as order forms) by other electronic and paper methods. All information is captured and stored as part of your “Contact Profile”. Your profile may include information like the following:

Cookies do not contain identifying information such as your name, but they can contain other identifiers that can be used to recognize you.
